To enroll your child at Blair Kids Academy, the following paperwork must be submitted:
Tuition Express Authorization Form (Download)
DHHS Parent Brochure (Download)
A current copy of your child’s Immunization Records
Medication Permission Form (if needed)

***Note A new registration form and payment agreement must be filled out for each program year or when switching from School Year to Summer and vice versa. All other paperwork can be transferred as long as the information is the most current available.
